Short Survey - IDHHC Wants Your Input!

The Illinois Deaf and Hard of Hearing Commission is seeking input from the Deaf, DeafBlind, DeafDisabled, or Hard of Hearing persons within diverse communities (including rural and urban) including any stakeholders or persons associated with the community residing in the state of Illinois.

Please take 5 minutes of your time to complete the survey. Your participation is 100% voluntary and the responses will be kept completely anonymous.  The survey includes ASL video with captions, transcript for the DeafBlind, and Spanish version (Versión en Español). Your responses to the survey can be submitted online, in an ASL video, or by appointment with IDHHC staff via telephone or videophone.
